Step 1 — Pay the One-Time Enrollment fee

  1. Visit modernedgefamily.com

  2. Go to the Services page.

  3. Select One-Time Enrollment Fee ($270) and complete checkout.

Step 2 — Choose Your Membership Plan

After paying the enrollment fee, return to Services and choose one plan:

Essential Plan – Core primary care membership

Select (monthly or annual).

Step 3 — If You’re on Controlled Substances

If you are prescribed medications such as Tramadol, Hydrocodone, or Clonazepam:

  1. Add the Controlled Substance Management plan.

  2. If you are on two or more controlled medications, add $5 for each additional by selec

Step 4 — Wait for Your Charm Health Invitation

Once you have completed:
✅ The $270 enrollment fee
✅ Your membership plan
✅ Any controlled-substance add-ons (if applicable)

Our team will verify your registration and email your Charm Health Patient Portal invite.
Use this portal to complete your intake forms, upload records, and schedule your first visit.

Important Notes

  • All steps must be completed before we can send your Charm Health invite.

  • Double-check your email address during checkout to avoid delays.
