🌱 Essential Care — $65/month

• Annual comprehensive visit (up to 60 minutes)

• Same-week appointments (office or telemedicine)

• Access to direct messaging for simple questions

• Office & telehealth visits included

• Does not include home visits

🌿 Enhanced Care — $75/month

Includes all of Essential Care, plus:

• Extended follow-up visits (up to 45 minutes)

• Monthly wellness check-ins (virtual or in-person)

• Weight loss & nutrition counseling included

• Priority scheduling for urgent needs

• Discounted member add-ons (pain management, IV therapy, advanced labs, etc.)

🌟 Signature Care — $110/month

Includes all of Enhanced Care, plus:

• 24/7 direct access for urgent concerns

• Quarterly wellness strategy sessions (60–90 min)

• Home visits or house calls available by request

• Personalized care coordination with specialists

• Advanced therapies included (peptides, HRT, pain management support)

Cash-Pay / No-Insurance Policy

Office visits for patients without insurance are $120 per visit.
Patients who do not have insurance are considered cash-pay and are required to pay the visit fee at the time of service.

In addition to the per-visit fee, all cash-pay patients are still required to pay the applicable enrollment fee and monthly membership fees, if enrolled in a membership or concierge-style program. Cash-pay status does not waive enrollment or monthly fees.

Fees are due regardless of insurance status and are not billable to insurance.
