🩺 Controlled Substances
Pain management/controlled substance patients are accepted at the discretion of the provider and based on individualized clinical assessment. A processing fee applies to controlled substances: $10 for the first medication and $5 for each additional controlled prescription.

Is there an enrollment fee?
No. We do not charge an enrollment fee.
Is there a minimum commitment?
Yes. All memberships require a six-month commitment. After that, memberships continue month-to-month.
What happens if my card on file declines?
You have 24 hours to update payment information. If not updated, services are placed on hold and a reactivation fee may apply.
What if my insurance is not accepted?
You may still enroll as a self-pay member. The monthly membership fee applies and visits are $120 per visit.
