Membership benefits require a 3-month minimum commitment. Annual memberships receive a 10% discount when paid in full. Insurance may be billed when applicable. Membership fees are non-refundable. To reserve your first non-member appointment, a non-refundable $99 enrollment credit is required. It may be applied to a membership package if used within 30 days of your appointment.

Controlled Medication Management
Patients requiring ongoing management of controlled medications are enrolled under the Signature Membership plan to ensure appropriate monitoring, follow-up care, care coordination, and compliance requirements.

Examples may include ongoing prescriptions for:
-Benzodiazepines (such as diazepam, alprazolam, or clonazepam)
-Sleep medications
-Stimulant medications
-Certain chronic controlled pain medications

This membership level allows for the additional time, documentation, monitoring, and follow-up required for safe and compliant prescribing practices.
