Modern Edge Family Practice offers personalized primary and wellness care, including but not limited to medical consultations, health screenings, immunizations, and wellness planning. All services are provided in Albuquerque, NM.

2. Appointments and Cancellations

Appointments are required and can be scheduled by phone or through our patient portal. Please provide at least 24 hours’ notice for cancellations or rescheduling. Failure to do so may result in a cancellation fee.

3. Patient Information

You agree to provide accurate and complete information regarding your health history and current conditions. All information shared with Modern Edge Family Practice is confidential and protected under applicable privacy laws, including HIPAA.

4. Communication

We encourage direct communication between patients and providers through secure channels. Responses may take up to 48 hours during business days. For emergencies, please call 911 or visit the nearest emergency room.

5. Payment and Insurance

Payment is due at the time of service unless prior arrangements have been made. We accept most major insurance plans; however, insurance coverage is not guaranteed. You are responsible for any charges not covered by your insurance.

6. Patient Responsibilities

Patients are responsible for attending scheduled appointments, providing complete and truthful health information, following medical advice, and notifying the practice of any changes in contact or insurance information.
